Q: What is a hybrid model?
A: It means we now offer both insurance billing (for qualifying plans) and cash-pay options. You choose what works best for your visit!
Q: Will you still take my insurance?
A: Yes, if your plan is in-network with us and you’ve met your annual deductible. If not, don't worry - we will securely hold a card on file and request a deposit at the time of service based on the visit.
Q: What if I haven’t met my deductible?
A: No problem! You can opt to pay our transparent self-pay rate instead.
Q: Can I get a Good Faith Estimate?
A: Absolutely. If you're not using insurance, we’ll give you a clear estimate before any service is performed.
Q: What if I change my mind?
A: You can choose a different payment method at any visit — we’ll just have you fill out a new form.
Q: Are there any forms I have to sign?
A: Yes — to keep everything legal and transparent, you’ll be asked to sign either a “Patient Chooses to Self-Pay” waiver or an “Insurance Opt-Out Agreement” depending on your situation. These protect both you and the clinic and ensure we stay in compliance with Arizona and federal guidelines.
